Avenue 360 Health + Wellness is a community health center in Houston, Texas, with multiple clinic locations (The Heights, Humble, Midtown, Memorial City, Montrose, Southwest, Spring Cypress, and a new mobile clinic). The organization provides comprehensive services including medical care, dental and oral health, behavioral health/mental health, pharmacy services, hospice care, and supportive services. For more than 35 years, Avenue 360 has operated Omega House, the first hospice home dedicated to end-of-life care for patients with AIDS in Texas. The Supportive Services team works with community partners to address housing, transportation, and nutrition needs, and provides substance abuse recovery support.
