Wabash Community Health Center

Wabash Community Health Center is a non-profit community-based health care provider located at 1123 Chestnut Street in Mt. Carmel, Illinois. The organization offers primary care and behavioral health services to all members of the community regardless of ability to pay. As of April 1, 2021, the center received Federally Qualified Health Center (FQHC) look-alike status from the Health Resources and Services Administration (HRSA). The center employs a team of four providers including a primary care provider, Casey Ikemire APRN, FNP-C, MSN, FNP-BC, who sees patients of all ages; a psychiatrist, Dr. Tejas Patel; and two psychiatric nurse practitioners, Logan Keepes and Leann Lear, who address emotional and behavioral health needs. Financial assistance and discounts for essential services are available based on family size and income.
