Founded in 1955 as the Adams County Mental Health Center, the organization has served the Adams County area for 70 years providing high quality, affordable mental health, educational, and rehabilitation services. They serve people of all ages and people with diverse needs, touching the lives of approximately 6,000 area residents annually. In 2024 Transitions was designated a Certified Community Behavioral Health Clinic (CCBHC).
