Transitions is an addiction treatment provider offering a continuum of care for individuals with substance use disorders and co-occurring mental health conditions. The program follows evidence-based practices with individualized treatment plans delivered across multiple levels of care. Services include medical and clinical intake/triage, residential rehabilitation with individual and group therapy, intensive outpatient (IOP) and standard outpatient services, targeted case management addressing employment and housing barriers, therapeutic community programming, recovery housing with case management support, and Casey's Law assessments. Treatment addresses substance use disorder education, mental health therapy, and co-occurring disorders through integrated counseling and behavioral health services. Transitions operates in Kentucky, Ohio, and Indiana and accepts multiple state Medicaid plans.
