Founded by Dr. Flynn in 2008 through the Joan Viteri Memorial Clinic at UC Davis School of Medicine, Transitions evolved through partnerships with Harm Reduction Services and SANE before becoming an independent non-profit organization in 2022. Transitions leads with a harm reduction mindset and accepts insurance including Medi-Cal to provide affordable, complete addiction care.
