Southern New Hampshire Health is a non-profit health system that has provided care for more than 130 years, serving communities in southern New Hampshire and northern Massachusetts. Southern New Hampshire Medical Center is a 188-bed acute care facility located in downtown Nashua with a medical staff of over 500 primary and specialty care providers. Foundation Medical Partners, a multi-specialty provider group affiliated with the health system, includes more than 300 providers in primary, specialty and immediate care serving thousands of patients in more than 70 practices. Services include preventive care and annual wellness visits, treatment for common illnesses and injuries, management of chronic conditions, and seamless referrals to specialists for coordinated care. Immediate care services are available seven days per week as walk-in care for non-emergency medical needs, with virtual visits also offered.
