What to expect when you callTo be considered for admission, you must have a state substance abuse psychosocial assessment. You can obtain an assessment at your local Mental Health Center or call Phoenix House at (205) 758-3867 for an appointment at no cost. If you already have an assessment, fax it to (205) 758-3803 attention INTAKE or email [email protected] for review. The admission coordinator will assist with next steps once criteria are met and a bed is available.
