Yakima Neighborhood Health Services is a comprehensive primary care provider committed to improving quality of life in their communities through accessible and integrated health and social services. The organization focuses on ending homelessness, providing health education and advocacy, and serving underserved and vulnerable populations including uninsured and disadvantaged individuals regardless of ability to pay. They work collaboratively with hospitals and community providers to maximize resources for patient services. The organization operates five primary care locations across Yakima County and has achieved Patient-Centered Medical Home Level 3 recognition at all locations, being the first Community Health Center in Washington state to receive this highest national level recognition. They also meet all rigorous federal program requirements from the Bureau of Primary Health Care, including proper staffing levels, effective management, and maintaining accessible, safe, and affordable facilities.
