Louisiana Health and Rehab (LHR) is a Baton Rouge-based organization comprising two main entities: LA Health and Rehab Center, Inc. (LHRC) and LA Health and Rehab Options, Inc. (LHRO). LHRC is a CARF-accredited provider offering substance use disorder treatment, mental health services, and support for individuals experiencing homelessness and HIV/AIDS. The organization operates multiple programs including Reality House, a residential treatment program for pregnant women and women with dependent children aged 0-12; Life Care Center, an outpatient center providing intensive outpatient and standard outpatient substance abuse treatment for adults and children; and FASD Screenings and Modifications to Treatment, a SAMHSA-supported initiative serving adult populations. LHRO provides home and community-based waiver services for developmentally, intellectually, and physically impaired individuals of all ages, as well as elderly adults. Founded twenty years ago by Soundra Temple-Johnson, LHR has been serving underserved populations across Louisiana with an emphasis on facilitating long-term outcomes and recovery.
