Horizon Health and Wellness is a non-profit integrated health care provider licensed in Arizona and accredited through The Joint Commission. For more than 40 years, Horizon has served Arizona communities with comprehensive health care services including family medicine, behavioral health, addiction treatment, psychiatry, counseling, and telehealth. As a Certified Primary Care Medical Home, the organization provides inpatient, outpatient, and residential services for all ages and stages of life. Their services include children's services, crisis services, domestic violence services, group therapy, HIV services, intellectual and developmental disabilities services, supported employment, and supportive housing.
