Heritage NW Group is a network of interconnected service providers based in Washington State that takes a holistic care approach to serve seniors, people with disabilities, and underserved populations. The organization consists of four main divisions: Heritage NW Consulting (person-centered behavioral and mental health services for Medicaid-eligible aging and disabled clients in partnership with the Washington State Department of Social and Health Services), Heritage NW Counseling (personalized mental health services for individuals and families with insurance accepted), Heritage Moving Solutions (specialized moving services for aging and disabled populations), and Heritage Home Revival (organizational, downsizing, decluttering, and home services). The group provides stabilization support services and works across multiple care settings including independent living, assisted living, adult family homes, memory care, and skilled nursing facilities, with a focus on empowering clients toward independence and maintaining dignity.
