FourPoints Health

FourPoints Health is a community health organization operating multiple clinics across Utah (Cedar City, Richfield, Kanosh, St. George, and Shivwits) that provides integrated primary care, behavioral health, dental, and optometry services. Their behavioral health division offers mental health counseling, marriage and family counseling, and substance use disorder treatment to individuals, couples, and families of all ages. They emphasize an integrated care approach addressing the body, mind, emotions, and spirit, rooted in Paiute cultural values. Services are available on a fee-for-service basis with sliding scale fees for uninsured patients and no charge for Paiute Tribal members and other Purchased Referred Care eligible Native Americans.
