Elevacare

ElevaCare is a community behavioral health center serving southwest Minnesota, including Nobles, Jackson, and Cottonwood counties. The center provides comprehensive whole-person centered care focused on optimal healing and recovery. They offer multiple levels of care including adult outpatient therapy, child outpatient therapy, psychiatry, assertive community treatment, intensive residential treatment, and crisis services. Additional programs include adult rehabilitative mental health services, dialectical behavior therapy intensive outpatient program, children's therapeutic services and support, adolescent dialectical behavior therapy, children's targeted case management, substance abuse treatment, urgent care, and mobile crisis response. Locations include Worthington, Windom, Jackson, and Lakefield, Minnesota, with an administrative office in Luverne.
