Call the Intake Coordinator at (541) 276-3518 Ext. 200 to begin the intake process, which includes a comprehensive Bio-Psych-Social Substance Use Assessment (ASAM), oral interview, urine analysis, belongings search, and document review before an individualized treatment plan is developed.
