Community Health Connections (CHC) is a multi-site, non-profit community health center serving patients in Leominster, Fitchburg, and Gardner, Massachusetts. As a Federally Qualified Health Center (FQHC), CHC offers primary family medical care, pediatrics, urgent care, dental services, behavioral and mental health services for children and adults, psychiatry, substance use disorder treatment, optometry, podiatry, and specialized clinics including TMS and Spravato.
