CHC of Cape Cod Addiction Program

Community Health Center is a comprehensive, non-profit medical home located on Cape Cod that provides primary care, behavioral health care, dental care, gynecology, optometry, recovery services, and specialized programs including Veterans care. The center offers on-site affordable prescriptions, x-ray and ultrasound services, and barrier assessments. As a Patient-Centered Medical Home recognized by the National Committee for Quality Assurance (NCQA), CHC provides coordinated, integrated care with access to physicians, nurse practitioners, and physician assistants. The organization serves all patients regardless of ability to pay or insurance status, with a discounted sliding fee schedule based on family size and income. CHC provides on-demand language interpretation services including American Sign Language. The center is committed to serving all members of the community without discrimination and offers care that reflects patients' cultures and beliefs.
