Bridging Access to Care is a community-based healthcare organization providing trauma-informed care in mental health support, substance use treatment, supportive housing, and prevention education. Originally founded as the Brooklyn AIDS Task Force in 1986, the first HIV/AIDS organization in Brooklyn, BAC transitioned in 2012 to a multi-service organization while continuing to serve persons living with HIV/AIDS and their families. The organization provides coordinated healthcare and supportive services to underserved racial and ethnic minority communities in Brooklyn and other parts of New York City, with services including outpatient substance misuse treatment, outpatient mental health treatment, case management, care coordination, harm reduction, HIV prevention education and testing, health home care management, housing placement assistance, and medication adherence support.
