Axis Health System (Southwest Colorado Mental Health Center, Inc.) is a community health center serving Archuleta, Delta, Dolores, Gunnison, Hinsdale, La Plata, Montezuma, Montrose, Ouray, San Juan, and San Miguel counties in southwest Colorado. The organization offers integrated whole-person care including primary care, behavioral health, dental care, and substance use treatment.
